County Health Rankings & Roadmaps

A program of the University of Wisconsin Population Health Institute, The County Health Rankings & Roadmaps program provides data, evidence, guidance, and examples to build awareness of the multiple factors that influence health and support leaders in growing community power to improve health equity. The Rankings are unique in their ability to measure the health of nearly every county in all 50 states, and are complemented by guidance, tools, and resources designed to accelerate community learning and action.

The Blue Cross Blue Shield Health Index is a health metric that identifies the top 200 heath condition categories affecting Americans, providing a view of the overall health and wellness of a population down to the county level. Powered by data from more than 45 million BCBS commercially insured members, the BCBS Health Index brings a unique contribution to other available health data to support national and local discussions about how to improve health, healthy behaviors, health policy and healthcare practice in the U.S.
